Health Benefits / COBRA
Health benefits will remain active through the end of the month in which employment terminates.
You should be receiving a COBRA election packet (including cost) from the COBRA administrator Navia within a few weeks after your termination processes in the system.
If you wish to continue coverage in the plan you are currently enrolled in, you will be covering both employee and employer portions of the premium plus a 2% administration fee, so you will experience an increase in your out of pocket costs.

Health Savings Accounts (HSA)
HSA funds are the employee's to keep even after termination, although employer contributions will cease.
Once the termination date is sent to HSA Bank, the terminated employee will be liable to pay the $2.25 monthly admin/service fee associated with keeping the account open.

One Medical
To be eligible for the sponsored One Medical membership, you must be enrolled in a Sequoia sponsored Anthem health insurance plan. Once the employee is no longer covered by a Coda Project Anthem plan, they will also lose eligibility for One Medical.

401k
If you need to rollover your funds, you should be able to do that from your profile or by contacting an agent. .

W2
You can still access the WorkLife portal with your username and password. If you need to reset your password, your personal email is affiliated with your Sequoia WorkLife account.

Employee Documents
If you need to access your personnel documents that was completed upon joining Coda, please reach out to [contact]

Last Paycheck
It depends on your residence State (some States require for you to be paid on your last day of work and in other States, you get paid on the next regular payday)
If you had direct deposit setup, that is an option for CA residents.
Please check with the People Ops team to confirm the date of your last pay day

Equipment
Our People team will be reaching out to you to arrange the return of your equipment.
